As kidney transplantation continues to evolve, a growing body of research suggests that robotic-assisted kidney transplantation may help expand access to minimally invasive surgery without compromising patient outcomes.
A new review published in the June 2026 issue of the Bulletin of the American College of Surgeons examined the latest evidence on robotic kidney transplantation and found that the approach achieves patient and graft survival rates comparable to traditional open surgery while reducing wound complications, hospital readmissions and recovery challenges for some patients.
The review highlights how robotic kidney transplantation, once considered a highly specialized procedure performed at only a handful of centers, is gaining momentum as more surgeons gain experience and evidence supporting its safety and effectiveness continues to grow.
"Kidney transplantation remains the best treatment for most patients with end-stage kidney disease, and our goal is always to make that treatment safer and more accessible," said Phillipe Abreu, MD, Director of Robotic Transplantation, Tufts Medical Center and assistant professor, Tufts University School of Medicine. "The evidence we're seeing suggests that robotic kidney transplantation can offer patients the same excellent transplant outcomes while potentially reducing some of the surgical burden associated with traditional open procedures."
Unlike conventional kidney transplantation, which requires a larger abdominal incision, robotic-assisted procedures are performed through several small incisions using robotic instruments controlled by the surgeon. The technique may be particularly beneficial for patients who face a higher risk of wound complications, including individuals with obesity.
As adoption expands, researchers are continuing to evaluate long-term outcomes and identify which patients may benefit most from the approach. According to the review, recent matched studies have demonstrated equivalent patient survival and kidney graft survival compared with open surgery, while also showing lower rates of wound-related complications and hospital readmissions.
"What's exciting is that we're moving beyond questions of feasibility," Dr. Abreu said. "The conversation is increasingly focused on how robotic transplantation can be integrated into clinical practice in a way that improves the patient experience while maintaining the highest standards of transplant care."
The findings come at a time when transplant programs across the country are pursuing innovations aimed at expanding access to lifesaving kidney transplantation. More than 90,000 people in the United States are currently waiting for a kidney transplant, underscoring the need for continued advances in surgical techniques and donor access.
At Tufts Medical Center, transplant specialists continue to advance living donor and kidney transplant programs through initiatives that expand access and improve patient outcomes, including participation in the Alliance for Paired Kidney Donation and investments in innovative transplant care.
"Robotic kidney transplantation is one example of how innovation can help us rethink what's possible for transplant patients," Abreu said. "As the evidence base grows, we're gaining a clearer understanding of where this technology can make the greatest impact."
