Advances in robotic surgery are helping surgeons perform complex kidney procedures through smaller incisions, offering another option for some patients who may benefit from a less invasive approach.
"Historically, patients with a higher BMI have faced additional challenges when undergoing major abdominal surgery," said Phillipe Abreu, MD, Director of Robotic Transplantation, Tufts Medical Center and assistant professor, Tufts University School of Medicine. "Robotic surgery allows us to perform these procedures through much smaller incisions, which can help reduce some of the risks associated with larger surgical wounds."
During robotic kidney surgery, surgeons control robotic instruments from a nearby console. The technology provides a highly magnified, three-dimensional view of the surgical field and allows surgeons to make precise movements in areas that can be difficult to access through traditional open surgery.
For patients, the biggest difference is often what they don't see: a large incision.
"One of the advantages of robotic surgery is that we're able to achieve the same surgical goals while minimizing trauma to the abdominal wall," Dr. Abreu said. "That can translate into less pain, fewer wound-related complications and a smoother recovery for many patients."
Research has shown that robotic kidney procedures can be performed safely in patients with obesity and even morbid obesity, with outcomes comparable to those seen in patients with lower BMIs. Studies have also found that obesity does not appear to significantly increase complication rates following robotic kidney surgery.
While robotic surgery is not the right choice for every patient, it has expanded treatment options for many people who may have previously faced additional barriers to surgery.
"The most important thing patients should know is that a higher BMI doesn't automatically prevent someone from being a candidate for kidney surgery," Dr. Abreu said. "Every patient deserves an individualized evaluation, and today's surgical technologies allow us to care for patients who may have been considered higher risk in the past."
