The following form is the first step in our comprehensive evaluation process. It is designed to help our transplant team determine if living donation is a safe option for you, as your health and safety are our top priority.

What to Expect
Confidentiality: All information provided in this application is strictly confidential and will only be reviewed by the Tufts Medical Center liver transplant team. It will not be shared with the potential recipient or their family.

Time Commitment
This initial questionnaire takes approximately 10–15 minutes to complete.

Next Steps
After you submit this form, a member of our Living Donor team will review your information and contact you within two business days to discuss the next phases of the evaluation.

Before You Begin
Please ensure you have your basic medical history and current contact information ready. You will have the opportunity to note the intended transplant recipient on the form.
