Additional Notes: The Ophelia process consists of three phases: (1) Identifying strengths, needs and action ideas, (2) Selecting, planning, and testing health literacy action, (3) Implementing, evaluating and improving health literacy actions. The first phase often involves utilizing the Health Literacy Questionnaire (HLQ) to assess the health literacy needs of patients. HLQ is a validated measure used for reflection on personal health literacy and experiences engaging with health information within the healthcare system. The second phase involves the development of a program logic model and Plan Do Study Act (PDSA) quality improvement cycles. The Ophelia protocol also includes guidelines for the evaluation of interventions. The eight Ophelia principles are as follows: (1) Outcomes focused, (2) Equity driven, (3) Co-design approach, (4) Needs-diagnostic approach, (5) Driven by local wisdom, (6) Sustainable, (7) Responsiveness, and (8) Systematically applied.
