A 75 item questionnaire across six dimensions of health insurance literacy including self report confidence in health insurance concepts, objective knowledge assessment, self assessment of HIL level, data on purchase and utilization of health insurance, and demographic information.

Scoring categories:
Scores for the objective knowledge assessment are calculated by summing the number of correct answers (maximum 23). Scores for understanding key insurance concepts were self reported across 11 concepts on a scale of 1-4 for each.
