Kelli True, BSN, RN Promoted to Director of Referral Management + Transitions of Care at Home Health Foundation

Lawrence, Mass.  – Kelli True, BSN, RN has been promoted to director of referral management and transitions of care at Home Health Foundation, a Lawrence-based nonprofit organization under the Wellforce Health System which provides home health, palliative, hospice, aide and private care services in all the places patients call home.

In her new position, True will play a strategic role in executing the organization’s marketing and referral growth plans.

Diane Farraher-Smith, MSN/MBA, RN, chief clinical integration officer of Home Health Foundation, praised True’s leadership and dedicated commitment.

“Kelli’s focus on the patient and the family in ensuring smooth transitions of care and optimal health care delivery is tremendous,” Farraher-Smith said. “Her ability to form strong medical, nursing and therapeutic relationships between providers in the best interest for the patient makes her the ideal professional for this role.”

True joined Home Health Foundation as a nurse case manager for Home Health VNA in 1999. Over the past six years, her roles progressed in responsibility from care transition liaison to senior care transition liaison and, most recently, care transitions manager.

True earned her BSN from Northeastern University. She is currently pursuing an MBA in health care administration/management from Fitchburg State University.

Even with nearly 25 years of experience as a registered nurse, True said she feels the same sense of commitment each day while working with her team to make a difference for patients and families throughout the region.

“I’m excited to take this next step in my career, with an agency that supports my personal values and goals,” True said. “I recently experienced the tremendous loss of my husband, which brought home what so many of our patients and families face when dealing with illness. I’m proud to be in this profession and work in this community, as I had envisioned so many years ago.”

True lives in Hampstead, N.H.
