Wellforce is pleased to announce that Jenny Chiang, MD, MS, FAAFP will join Wellforce as Medical Director for the Wellforce Care Plan. Dr. Chiang is currently the Medical Director at MetroWest Healthcare Alliance and is a member of the NEQCA Board of Trustees.
While at MetroWest Healthcare Alliance, Jenny has been focused on population health management across multiple risk-based contracts and on helping physicians shift toward value-based care for the past several years.
Because Jenny has made a tremendous impact at MetroWest Healthcare Alliance, she will also continue her current role as Medical Director.
Prior to joining MetroWest Healthcare Alliance, she worked with a large Medicaid population at the Greater Lawrence Family Health Center, where she was on the faculty with the Family Medicine Residency Program. There she practiced full spectrum Family Medicine, including outpatient pediatrics, prenatal care to term, inpatient pediatrics and newborn nursery rounding at Lawrence General Hospital, and designed the curriculum for Geriatrics and Palliative Care with focus on reducing polypharmacy and deprescribing.
As Medical Director of the Wellforce Care Plan, Jenny will bring her dynamic and collaborative leadership style to improve quality of care and patient outcomes and will be an important leader as we form our clinically integrated network. She will also work closely with our participating physicians at Lowell General Hospital, Lowell Community Health Center, Lowell General PHO, MelroseWakefield Hospital, Lawrence Memorial Hospital, NEQCA and Tufts Medical Center.
