2. Complete intake form - with your name, address, telephone number, email and insurance information
3. Intake form must be completed and submitted for someone to contact you within 5 business days to set up your first appointment
4. Download and fill out the medical and behavioral questionnaire
5. Bring questionnaire with you to the initial consult
